Ways to enroll a new Customer or Member into Life Force:
1. Online
- Go to www.mylifeforce.net
- Enter your Member ID number and Password
- Select "Enroll new Customer/Member" from the links on the left hand side
- Follow the prompts to complete the online enrollment form.
2. Phone
- Call Customer Service at 1-800-531-4877 with your new Customer/Member on the line with you.
- Allow our friendly Customer Service Representatives to assist you.
3. Fax
- Fill out a New Member/Customer Registration Form and fax it to Customer Service at 1-800-809-8208.
If you don't have a registration form, you can download the form here:
- Go to www.mylifeforce.net
- Enter your Member ID number and Password
- Select "Member Services"
- Select "Document Library"
- Select "Registration Form" for the country you require
- Print the form
- Fax the completed form to your local office
